Lost Stolen Passport

How to Replace a Lost or Stolen Passport

A Lost or Stolen Passport replacement follows the same basic Form DS-11 procedure as a New Adult Passport, with one important additional step: a Passport that is lost or stolen must be reported only if it is still valid.

Follow each step in order. Bring every required document to your Passport Acceptance Agent Appointment.

I. Report the Lost or Stolen Passport (Only If It Is Still Valid)

If your U.S. Passport is lost or stolen and is still valid, report it immediately.

For the Legal Passport process:

  • Complete Form DS-64 — Statement Regarding a Lost or Stolen U.S. Passport.
  • Print the completed Form DS-64.
  • Sign Form DS-64 immediately, before your Passport Acceptance Agent Appointment.
  • Bring the completed and signed Form DS-64 to your appointment.

Do not report an expired Passport as lost or stolen.

Once a valid Passport has been reported lost or stolen, it is cancelled and cannot be used again even if you later find it.

Damaged or Mutilated Passport

If you still have the damaged or mutilated Passport, do not report it lost or stolen.

Apply for the replacement using Form DS-11 and bring the damaged or mutilated Passport with you.

A SIGNED STATEMENT IS REQUIRED

Prepare and sign a written statement explaining how the Passport became damaged or mutilated.

Bring:

  • The damaged or mutilated Passport
  • The signed statement explaining the damage
  • The other documents required for the Form DS-11 application

The State Department specifically requires the damaged Passport and a signed statement explaining its condition.

II. Complete, Download, and Print Form DS-11

Complete Form DS-11 using the U.S. Department of State online form filler.

  • Type your information into Form DS-11.
  • Download the completed form as a PDF.
  • Print the completed barcode form.
  • Confirm that the large 2-D barcode appears near the top of the application.
  • Use the same international departure date shown on your confirmed itinerary.

Do not sign Form DS-11 before your Passport Acceptance Agent Appointment.

Sign Form DS-11 when instructed by the Passport Acceptance Agent.

III. Proof of U.S. Citizenship

Bring one original or certified physical document proving U.S. citizenship, such as:

  • Certified U.S. Birth Certificate
  • Previous U.S. Passport
  • Certificate of Naturalization
  • Certificate of Citizenship
  • Consular Report of Birth Abroad

Bring the original or certified document to your Passport Acceptance Agent Appointment.

IV. Photo Identification

Bring a valid physical photo ID, such as:

  • Driver’s License
  • State Identification Card
  • Government-Issued Photo ID

V. Passport Photograph

Bring one recent Passport photograph that meets U.S. Department of State Passport photo requirements.

VI. Proof of International Departure

Bring one copy of your confirmed international travel itinerary.

The travel date on your itinerary should match the departure date entered on Form DS-11.

Your application can be submitted by the Courier only when your travel qualifies for Passport Agency processing.

VII. Legal Passport Letter of Authorization (LOA)

The Legal Passport Letter of Authorization is required for Courier submission.

Complete the Legal Passport Letter of Authorization (LOA) provided with your current order.

  • Enter your name as the applicant.
  • Write Legal Passport in the Courier company field.
  • Sign the LOA with your original signature in black or dark blue ink.
  • Include the original signed Legal Passport Letter of Authorization with your application documents.

Do not substitute a Letter of Authorization from another company.

VIII. U.S. Government Payment

Prepare a check or money order for:

$213.36

Make it payable to:

U.S. Department of State

The $213.36 includes:

  • $130 Passport Book fee
  • $60 expedited processing fee
  • $23.36 for 1–3 day return delivery

Write the applicant’s full name and date of birth in the memo section.
