National Passport Specialists

If the U.S. Department of State made a data or printing error on your passport, the error can be corrected.

Common passport errors include:

  • Misspelled name
  • Incorrect date or place of birth
  • Incorrect information on the biographical page
  • Missing information
  • Printing discoloration
  • Crooked or defective printing
  • Other government data or printing errors

48HourPassport.com can help travelers prepare a correction application and coordinate urgent Courier submission when qualifying international travel is approaching.

Government Error or Legal Name Change?

A data or printing error is information the government printed incorrectly.

For example, your birth certificate may show your last name as Anderson, but the passport was printed as Andersen.

That is a data error.

A legal name change is different.

If your passport correctly shows your former legal name and you later changed your name because of marriage, divorce, or a court order, follow the Legal Name Change procedure instead.

Form DS-5504

Passport data and printing errors are generally corrected using:

Form DS-5504 — Application for a U.S. Passport for Eligible Individuals

Complete the current government form and follow the instructions provided with your 48HourPassport.com order.

What You Need

A passport correction application generally requires:

  • Completed Form DS-5504
  • Your current passport containing the error
  • One current color passport photo
  • Evidence showing the correct information
  • Proof of international departure for urgent processing
  • Legal Passport Letter of Authorization for Courier submission
  • Any additional documentation required for your particular correction

Evidence of the Error

Provide documentation showing what the correct information should be.

For example, if your last name was printed incorrectly, appropriate evidence might include your certified U.S. birth certificate showing the correct spelling.

Other supporting documents may be appropriate depending on the type of error.

The documentation should clearly establish that the information printed in the passport is incorrect.

Your Current Passport

Submit the passport containing the error.

The State Department needs the incorrect passport in order to issue the corrected replacement.

Do not destroy, alter, write on, or attempt to repair the passport yourself.

Passport Photo

Submit one current color passport photograph meeting State Department requirements.

The photograph should:

  • Measure 2 × 2 inches
  • Have been taken within the last six months
  • Show your current appearance
  • Have a white or off-white background
  • Meet current U.S. Department of State photo standards
  • Not be digitally or AI altered

Government Fee for Correcting an Error

If the U.S. Department of State made a data or printing error and the passport is still valid:

There is no government application fee to correct the error.

If expedited processing is requested, the current expedited service fee is:

$60

Optional 1–3 day return delivery of the corrected passport book is currently:

$23.36

These optional charges are separate from the no-fee passport correction itself.

How Long Will the Corrected Passport Be Valid?

The timing of the correction affects the validity period of the replacement passport.

Error Reported Within One Year

If you report the government error within one year of the incorrect passport being issued:

  • An adult’s corrected passport will generally be valid for 10 years
  • A child’s corrected passport will generally be valid for 5 years

Error Reported After One Year

If the error is reported more than one year after the incorrect passport was issued, the corrected passport will generally expire on the same expiration date as the original incorrect passport.

The correction does not restart the passport’s validity period.

Do You Need an Acceptance Agent?

A standard DS-5504 correction does not normally require an Acceptance Agent appointment.

This is different from a DS-11 application.

If your circumstances require a different passport form, additional procedures may apply.

Proof of International Departure

For urgent Passport Agency processing, international travel normally must be within two weeks.

If your destination requires you to obtain a foreign visa before travel, qualifying travel may be within four weeks.

Proof of upcoming international departure is required for urgent processing.

Check a New Passport Immediately

When you receive a new passport, check the information immediately.

Verify:

  • Full legal name
  • Date of birth
  • Place of birth
  • Sex marker
  • Passport photograph
  • Passport expiration date
  • Passport number and biographical-page printing

Finding an error immediately is much better than discovering it shortly before an international trip.

Printing Problems

A passport printing error can include problems such as:

  • Missing information
  • Discoloration
  • Crooked printing
  • Defective printing on the biographical page
  • Other production errors affecting the passport

Do not attempt to fix a printing problem yourself.

Have the passport officially corrected.
