For Individuals who:
- Have a data error on their current U.S Passport,
- Have a printing error on their current U.S Passport, or
- Have made changes to their sex or gender identity.
I. Proof of Data Error
If there is a data or printing error on your passport, you can have it corrected. A data error may involve issues with your name, gender, or place of birth, while a printing error may include missing data, discoloration, or crooked printing on the biographical page. The Passport Agency will correct the error free of charge if the passport is still valid.
To report the error, you need to provide evidence of the mistake, such as your certified original U.S. birth certificate showing the correct spelling of your last name.
If you report the error within one year of receiving the incorrect passport, the new passport will be valid for 10 years.
If you report the error after one year, the new passport will be valid until the expiration date of your original incorrect passport.
II. Photo Requirements

III. DS-5504 Form - Name Change, Data Amendments, Errors
To request a name change, report a data or printing error, or provide supporting documentation, the applicant must complete a DS-5504 form. Please ensure all necessary details and documentation are included.
IV. Authorization Letter
Please complete one (1) Letter of Authorization with original signatures.
IMPORTANT: The Letter of Authorization (LOA) must be filled out and signed by the applicant in black ink. The LOA should be placed inside the Government envelope. The name of the courier company is required by the government and should be printed or written in black pen on the line above where you sign your name.
V. Your most recent U.S. Passport
Please ensure that your most recent U.S. Passport is in good condition and not damaged or mutilated.
VI. Call to Reserve a Courier, Make Payment, and Receive the Shipping Address
To reserve an appointment with a courier at the Passport Agency for your application, please call (800) 238-6662.
Payment must be made to 48Hour Passport by credit or debit card only. In order to reserve a time slot with the US Passport Agency, please provide your credit card information. Keep in mind that this payment is non-refundable, as the Passport Agency has limited reservations. Payment is required to secure your reservation and guarantee expedited service. It's important to note that eligibility for a passport is determined by the US Passport Agency based on proof of citizenship and compliance with the provided instructions.
