For Individuals who:
- Have undergone a name change within one year of their passport being issued, or
- Have made changes to their sex or gender identity.
I. Proof of Name Change
To provide proof of a name change, you can provide an original certified legal name change document such as a marriage certificate, divorce decree, or court order.
II. Proof of Identification
Either a current and valid driver's license or a state ID, both featuring a photo, that has been in effect for at least a year.
If you lack both of these forms of identification, you will need to provide Supplemental Identification.
Applicants who are 16 years old must have one parent co-sign their application.
Applicants who are 17 years old may need one parent to co-sign if they have not possessed a driver's license or state ID for the past year.
III. Photo Requirements
One passport photo with a white background, measuring 2×2. Eyeglasses are not permitted in the photo.

IV. DS-5504 Form - Name Change
To request a legal name change, the applicant must complete a DS-5504 form. It is important to provide supporting documentation that verifies the requested or reported amendments.
V. Authorization Letter
Please complete one (1) Letter of Authorization with original signatures.
IMPORTANT: The Letter of Authorization (LOA) must be filled out and signed by the applicant in black ink. The LOA should be placed inside the Government envelope. The name of the courier company is required by the government and should be printed or written in black pen on the line above where you sign your name.
VI. Your most recent U.S. Passport
Please ensure that your most recent U.S. Passport is in good condition and not damaged or mutilated.
VII. US Government Fees
You must provide a Bank Cashier's Check or Money Order made out to the U.S Department of State for $81.36.
VIII. Call to Reserve a Courier, Make Payment, and Receive the Shipping Address
To reserve an appointment with a courier at the Passport Agency for your application, please call (800) 238-6662.
Payment must be made to 48Hour Passport by credit or debit card only. In order to reserve a time slot with the US Passport Agency, please provide your credit card information. Keep in mind that this payment is non-refundable, as the Passport Agency has limited reservations. Payment is required to secure your reservation and guarantee expedited service. It's important to note that eligibility for a passport is determined by the US Passport Agency based on proof of citizenship and compliance with the provided instructions.
XI. FedEx Shipping
Here is the final checklist for what you need to Overnight FedEx:
1. Signed Copy of the letter of Authorization
2. DS-5504 with a bar code on the top left margin
3. US Passport in Good Condition
