Call 48HourPassport at (833) 220-0002 to reserve a slot with payment.
you will receive email with steps that need to be completed for each passport type.
Common Forms you will need for shipping to legal Passport Process Center.
DS-11 Form New Passport or DS-82 Form Renewals. Include DS64 Form for lost or Stolen Passports.
The Government form must be filled out online and printed with Barcode.
Government forms must be signed before shipping your documents. Failure to sign your documents will delay your processing time to getting your passport.
You must include Proof of Travel Itinerary or Company Letter. (Travel Itinerary can be travel statement or Airline Confirmation of travel with Dates.)
One Passport Photo. (This can be taking with our Partner A+ Mailbox & More LLC)
One Signed Letter of Authorization. (To be provided by legal Passport by email)
Cashier Check or Money Order payable to "US Department of State" for Government.
Please note any service fee paid to 48HourPassport does not include any Government fees for the passport. Any fees paid to 48HourPassport are non refundable as services starts from time you order to the time your application is submitted to the US Department of State Passport Office
